(I-693) Immigration Physicals

When you petition to register as a permanent residence or to adjust status. then typically you must undergo an immigration medical examination and receive vaccinations against vaccine-preventable diseases. If you are required to undergo an immigration medical examination, then we are here to make it a quick and simple process.

We will provide the I-693 form for you so the only things that you will need to bring at the time of your visit are:

• Government-issued photo identification. Such as a valid Passport, Driver's License, Green Card, or State ID. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit. 
• A record of all the vaccinations or immunizations that you have received.
• Some form of payment. Cash is preferred. You can pay through check, credit, or debit card but these have an additional 4% charge through our banking system.
• (Optional) If you would like to speed up your appointment you can also go to our Forms and Resources tab and print then fill out our new patient packet so that you don't have to do it when you come to the office.

 The immigration medical examination will be a review of your medical history and a physical examination.

Dr George Gindi will complete a comprehensive examination that will include a review of your medical history and a physical examination. After which he will test for communicable diseases such as tuberculosis, syphilis, and gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC). This is done through a series of blood tests. The CDC sets the requirements for the immigration medical examination which we follow exactly as they are set out to ensure that your process is successful.
